Service Description
*In-depth 75 minute evaluation in the office for functional history & exam, personalized treatment recommendations and extensive initial lab panel evaluation. *In-office follow up visits (1) per month to evaluate progress and adjust treatment plan *Phone Visits or Virtual Visits for on-the-go convenience ( 4 total per month). *Unlimited simple messaging or questions through EMR portal. (Complex or extensive require virtual visit and if beyond the 3 allowed will include an additional charge). *20% discount on quality medical grade supplements using convenient drop-ship online ordering. *10% of our IV formulations. *Collaboration with your primary care doctor and specialists so your medical team is all on the same page *Lifestyle plan to include mindfulness, habit building, exercise, and dietary instruction *FREE admission to sit in any live workshops when presented in office. *Access to a diet plan specifically designed for your health concerns including recipes for starter week an handouts for shopping and examples. **Health coaching with 30 min consult with our health coach and one 10-15 minute phone call per week/4 per month to check in on accountability with your personalized plan and help support you and trouble shoot where you may be needing help.
Cancellation Policy
There is nonrefundable booking fee if cancellation or no show due to volume needing to be scheduled. Generally any labs and forms need to be completed about 1 weeks prior to appointment. I will be spending several hours going over forms and labs prior to consultation. Consultation includes recommendations for nutrition, supplements, movement/exercise, self care/mindfulness, clearing toxic burden, IV therapies and referrals if needed.
